For starters, replace "CREATURE:Golem" with "CREATURE:GOLEM", otherwise it will look for the creature with the creature token Golem which doesn't exist. If a creature can't be found, it defaults to TOAD. After that, clear the error log and then generate a new world, post the errors here.
I put the creature in the amphibian folder
If you put the creature in an existing creatures file, you should NOT have

I didn't actually check the raws yet. First off, fix what I mentioned above, then clear the log, gen a world, post errorlog here and I could take a look at the raws. :)

Before testing your creature be sure to clear the error log.
Request: Put Error Log in
To do so put it in between the ][ in [-code-][-/code-] and remove the hyphens within the brackets.
The code from StagnantSoul must be placed after the [BODY:blahblahblah] tags to work properly.
Proper Body Detail Plans can be found in b_detail_plan_default, as you can probably guess ICE isn't in there.
Some of the tokens you are using are from this version (leads to wiki) (http://dwarffortresswiki.org/index.php/40d:Creature_token), while this is the current version (also leads to wiki) (http://dwarffortresswiki.org/index.php/DF2014:Creature_token). As you can see many tokens and tags are gone. Some have new incarnations. Positive DAMBLOCK creatures used to take less damage, fractional GENERAL_MATERIAL_FORCE_MULTIPLIER are hit with that percent(or fraction) of the force from attacks and falling (1:2 is half, 1:10 is 10%, etc). Negative DAMBLOCK took more damage, GENERAL_MATERIAL_FORCE_MULTIPLIER greater than 100 as a percent(or 1 as a fraction) are hit with more force by attacks and falling (ie 2:1 is 2x, 3:2 is 1.5x, 25:2 is 12.5x, etc).
