This is something I take quite seriously, I've got bad eyes, my hearing is messed up, a shoulder that barely works and two bad knees. My mind however has always been as sharp as a scalpel.
Which brings me to my current problem, and as a Marine who's never been scared of anything... It scares the hell out of me. My mind is going. It started small, losing what I was trying to say in mid sentence, forgetting a word just as I'd say it. Then it started feeling like I was pushing a car uphill to follow a cohesive line of thought for anything more than about 30 seconds. I'm writing this once sentence at a time, while my mind goes blank in between despite my attempts to stay focused. The people I work with I can't make understand, attempting to do brain surgery with a crowbar would be a very crude metaphor for it.
Any suggestion other than 'go to a doctor' would be appreciated. Only docs I have available are at the VA clinic (Veterans Affairs) who are... well let's just say they're usually on par with the bottom 5-10% among doctors. Helluva time of yearto have this to deal with.
P.S. it took me about half of the original Die Hard movie to manage to write this. Credits are rolling as I type.

I've heard Fish is good for the mind and that some thinking problems are caused by a deficiancy of certain nutriance that you almost only get in fish.
You can also improve your memory with memory games. After a certain age (around 20) your brain starts to go downhill and it will begin to lose ability.
I can't say I've experienced what you have too much because I have natural thinking issues. Namely my brain organises information through unrelated corelations rather then linear thinking making my mind more like a maze then a web (I am not stupid, it just means I have information gathering and organisation problems).
Ohh I do have something similar. I have a learning disability where occasionally my brain can drop information I hear for little-no reason (It is as if I didn't hear it). It gets pretty scary when I notice it afterwords. Either that or the fact that I used to be an excellent Essay writer but lately my essays have all been mud boarderlining on gibberish.

Honestly, when you get to the point where you can't keep a coherent thought, going to a doctor is probably your best bet. Other than that, try telling someone you can talk to face-to-face, they should be able to help you more than we can here.

Weird, I used to have the exact opposite problem with hearing things. random comments would get lodged into my long-term memory rather indirectly. Any time I did what I was doing when I heard it, my mind would replay the comments. I can't remember that having happened in quite a while, so maybe this ahs been longer in sneaking up on me then I think?

Mate... if I had someone to talk to I would, but my options are limited to an exwife who can barely stand me or a 4-year old. (Insert self-explorative/bitterly ironic comment here) And yeah, I know what I need to do... I just don't want to lose my son in the process. Defeats the point.

So, I can't really say that I have the same problem, given that it's less like "is going" and seems more like "was never there." I constantly forget what I'm thinking about, and end up repeatedly saying "I'm thinking about something. What am I thinking about? I know I'm thinking about something." A habit of making up words and repurposing them in rather eccentric ways, etc., because I can't figure out how to say things "the usual way." I also often have a hard time remembering short lists or what people told me just a couple of minutes ago.
I'm not sure about the concentration problems, but my best guess is stress--all of my issues around this act up much worse when I'm stressed. I can also tell you that if the forgetting words and trains of thought is permanent, it doesn't mean that you're stupid or that you can't function well. It just means that you have to train yourself to hunt down what you were just thinking or the given word or the list of things you were supposed to do. Even at the best of times I seem to constantly forget what I'm in the middle of speaking or thinking, and may spew out reams of incomprehensible information. I'm accustomed to that by now, though, and know what it feels like when I've turned off my "inner translator" and start speaking Vector-ese... so I pause for a while, manually turn the translator back on, and figure out how to re-iterate what I meant in English.
Just be patient with yourself. It isn't so bad.

It could be stress, which can be fixed by relaxing a bit.
Or it could be a psychological problem maybe, so you may want to hunt down any unfixed problem or loose ends in your recent life.
Or it could be something bad happening to your brain. I would suggest fixing your doctor problems, then getting some tests done to see if there is anything wrong.
also, wild guess: have you perhaps changed your sleeping schedules lately? messing with sleep may be the cause. Unluckily, only you know exactly how it feels, so it may be hard to understand for other people.

Could be schizophrenia, some of the varieties involve "negative symptoms" where your thoughts and speech become slow or disorganized, as opposed to positive symptoms where you hallucinate or have delusions. See a psychiatrist in any case ASAP

Hmm, the way you put it it sounds like an acute problem that came up rather suddenly. It could be anything from stress to minor stroke, so it's best not to take chances and just go see a doctor. Crap doctor is still better than no doctor, and he may refer you to a specialist who should be a decent doctor.

Well, to be honest, at the extreme it could be a small brain tumor, though patients with neural cancer seem to get very hostile towards loved ones, and you seem to not be that way-It might be immense loads of stress, if so I'd recommend using a stress-toy or gum? I find gum relaxing.
P.S./Edit:Also, you should go to your local doctors anyway. They'd probably transfer you to a hospital and even if not, its better then dieing.

There is a hand test for brain cancer isn't there? You test your thumbs reflexes?

When it comes to brain stuff, don't fuck around if you don't know what causes it.
The proper term is "Psychiatric hospital" Which I think is for whimps.
95% of the people in a "Psychiatric hospital" have issues like unstopable stuttering, easily distracted, always depressed, rambling incoherently, and forgetfullness.
They deal with brain issues on a regular basis. That includes everything you just described, except the knee-issues.
Note that some specialize in treatments, others speciallize in housing check-ins with more permenant problems.

Go to a good docter (specalist). Don't have one in your area? please don't tell me your going "oh my brain is going, but its too far to drive man!". Its more important than that mate.

That's a little older than the usual age of onset of schizophrenia, but not impossible.
Of course, there's a shitload of other things it could be, and all of them merit him going to a psychiatrist right now.
