I am plagued by flies. Not bigtime but I just can't get rid of them. Cats don't kill flies and they can't be trapped so how can I deal with them? Are they blocked by doors? Do they move in all 8 directions?

All vermin teleport semi-randomly, so they're able to flit though doors if they move in the right direction. Flies in particular will just temporarily spawn near whatever's attracting them (food in the open, rotting parts, etc). Make sure everything that's exposed is put in a bin, and put the refuse pile outside if it's safe/your entrance can take the traffic (alternatively, consider making a back entrance with an emergency floodgate).
If flies are just floating about you'll probably just have to deal with them. Note that vermin which move into webs will get killed (I'm not sure if this applies to swarms, but I think it does. I'm not sure what would happen with remains in that case).

Are they occurring near miasma-producing areas, or elsewhere?
It's probably that the odd piece of food or two was left to rot outside of the kitchens/in the meeting hall (interrupted/resassigned/lazy dwarves drop stuff all the time) and was never picked up another hauler.
Some flies are always unavoidable but...I always make up and down ventilation for my kitchens, tanners and butcheries. Can move up and down too, and most of them tend to get lost in my ventilation systems before going poof.
First though make sure you have enough people doing food hauling, so prepared food isn't rotting for lack of transport to a barrel. Also make sure you have enough refuse dumpers assigned (and not bogged down by other tasks) so food that has rotted gets picked up and tossed asap, before it starts producing flies.
I also make my master-dump pile (I always make my interior) several levels deep, and ventilated but unconnected to the rest of the ventilation network.

I once tried a game where I made cats give alot more, so that they could be used in my meat industry. It worked, so I did catsplosions to end my fortress.

I've always wondered if the [FLIER] token is what makes them immune to cats and what would happen if you made a flying [VERMINHUNTER] (some kind of bat or small hawk).

I always dig all the way down from the surface to somewhere deep in the ground, then use that place to dump my refuse.
It rots underneath the sky, producing no miasma, and, most importantly; It doesn't make my dwarves unhappy.

I think that'd be an interesting idea, having cats able to kill/disperse flies. My dog eats flies.

It's probably the [VERMIN_MICRO] token that does it - dragonflies and monarch butterflies both have [FLIER] and cats are more than capable of capturing them.
