Well done! I was working on it in this thread http://www.bay12forums.com/smf/index.php?topic=146143.0
Looks like I was 10 off on the races offset which is why everything was breaking.
splinterz what tools do you use to find the memory offsets?
Right now I use a mix of QT debugging, WinDBG, Cheat Engine, and DF 25.00 (for corrected/uncorrected and vectors).

It works, but very sporadically.
Offsets have changed with skills and labors (since gelding) so it's buggy as it's not parsing gelding and other features yet.

I've been working on the 40.19 memory layout for Dwarf Therapist. I'm starting a thread in hopes of other people helping finish it.
Here's the current state: http://pastebin.com/hnVNE2Zg

Working:
Full dwarf names
General civilization information
All labors but gelding
Squads
Time/Date

Not Working:
Everything else

While I get the pull request completed for these on the official therapist branch, here are memory layouts for 40.13!

Win: http://pastebin.com/m8eYxan8
Mac: http://pastebin.com/nFkY1pb8

For win: Paste contents into a new ini file in etc\memory_layouts\windows of your dwarf therapist directory and you'll be ready to go
For mac:  go into your applications folder, ctrl+click therapist, and then open package.
Once package is open navigate to Contents->MacOS->etc->memory_layouts
Copy v0.40.10_osx.ini and rename it v0.40.13_osx.ini, open it in TextEdit and paste in the contents of the pastebin.
Happy therapizing!

I added a memory layout to Mac since it hasn't been done yet. You can find it on the original post

Since I don't know when the therapist repository will be updated I sat down and did my own memory layout for Windows 40.12
Copy the contents of this link: http://pastebin.com/wyL2wW2Q into a new ini file in etc\memory_layouts\windows and you'll be ready to go
Enjoy the therapy!

Update:
I added Mac layouts. You can find 40.12 memory layout for Mac here: http://pastebin.com/s6QzYnuS
To add it in go into your applications folder, ctrl+click therapist, and then open package.
Once package is open navigate to Contents->MacOS->etc->memory_layouts
Copy v0.40.10_osx.ini and rename it v0.40.12_osx.ini, open it in TextEdit and paste in the contents of the pastebin.
Happy therapizing!
