Bump for having the EXACT same problem, and finally managing to fix it.

Armok knows why, but the daffy bastard unwitting idiot dwarf in question managed to somehow teleport himself down 50 levels of unexplored cavern. and since he wasn't revealing any of it, he was well and truly lost.

if you feel like doing so, the quick fix that worked for me was using "DFHack" and the reveal command. unreveal can be used if you want to go back to the way the map was when he comes back.

a quick use of fastdwarf 1 1 to have him teleport back then fastdwarf 0 to set it back to normal.

Bump for hopefully answering this to anyone also having this problem. Firstly, I figured I'd check the Depot Access to the gathering site and what do you know; that area is labeled as unavailable :P. built a bridge over a river and they all came home :D.
