Maybe you have a pit or container underneath the latrine, which, after reaching a certain capacity, begins to produce miasma. Then of course, you could block off the room with a door, but using that latrine would be an unpleasant experience after too much sustained use. Thus, in the early stages of a fort, public latrines would work for the small populations. (Waste gradually decays, of course).
But then, as the fort grows in population, the latrines would obviously be full and leaking out huge amounts of miasma. Of course, the solution to this would be to move the waste to somewhere else, which would thus lead to the development of a sewage system.
A dwarf having a latrine in his/her room would generate a happy thought, and allow the player to develop an even more complex sewage system.
I do think this should be something that can be turned on and off, however.
