Designed, organised.
With workshop levels, Living level, military level, all separed by rooms of different industries. (Exemple, I have a room for stocking raw food, a room for kitchen/raw food, a room for alcool/still with barrels, a room for prepared food)   
All rooms are designed before start digging.
Exemple : Here, the entrance to my workshop z-lvl (each stair = one industrie, stone and gem, wood, cloth, and metal.) And my hospital. And maybe noble rooms.

I alway abandon because boredom (or because it disorganized). I spend more time making designations than playing. And I alway forgot some workshop (soap maker, my eternal nemesis). I'm a maniac.

You're dwarf get protected of the syndrome by clothings (yea, the pig tail sock is really effective).
Kittens and chield (who are, uh, naked) will rot and die. What is a good thing (or not).

Stockpile have quality levels, if you forbid the quality ''artefact'' of all stockpile, and if you create a custom stockpile who accept only artifact level.

If we are a god, we can't lock/unlock door in our fort, I think.

I don't think we are Armok. After all if I was Armok, badgers and other nasties would explode when they entered my embark site and cotton candy would materialise out of thin air. I think we're either a lesser God or the dwarven aspect of Armok

You can maybe with modding the blood temperature of badgers..

No leader, but caravans and migrants.

Your civ is alive, I think. It just don't have king/leader.

Starving migrants to death without them getting into my fort will not affect any Dwarves within my fort, will it? Not too fun of a tantrum spiral.

No, your dwarfs will be okay.
But, don't forget how dwarfs are thoughs. They can hunt vermin and drink water from pools and rivers. Living near the insanity, in the front of yours doors. Any dwarf who go outside can be attacked (or, starting a conversation and becoming friend.)

Migrants are more feared than goblins...

If this desired in dwarf fortress file depot, I think I will upload it. (I have a save with the map undigged, for savescumming.)

I want to build a ''boss room'' in my next fort.
One giant designed arena with a small lake in the middle. I will block the siege in it with retracting bridge.
Before sending the fierces warriors of the Insanity council from the ceiling!
Or the bronze colossus, if a catch one.
