I have made a lavaduct -a row of lava, one tile thick, hemmed in by two walls one tile thick. Ramps provide access for wagons across the top of the lavaduct. When the human caravan came, they rightly chose to move over the lavaduct, but when the center tile of the wagon (that seems to decide it's elevation) was on the ramp tile, the wagon extended into the wall in front of that, and the two horses pulling the wagon were... in the lava. Cue red legs, burning horses, a trail of smoke to my trade depot, and a HUGE fire due to all the logs I had requested.

I tried to find this hilarious bug on the list, but couldn't.

On the contrary. Ballistas are excellent training platforms, since you can save on ammunition by having them shoot at a wall with a moat directly in front of it. The ballista arrows will fall into the moat without breaking.

Same is also true for crossbow bolts, by the way, but since they currently will not go back together into a stack, saving them like this is pointless unless you wish a thousand hauling jobs at once.

All my champions have that trait, and they become about as unfeeling as Elves with it. I've recently sent the champions of my recent fort, who are breeding like bunnies, to clear out an underground lake. They slaughtered the olmmen and what have you, but when I inspect the carnage I see two babies floating in the water. "No biggie, they'll be picked up now". Right? Wrong. One of the babies eventually matured into a child and clambered out of the pool by itself, but the other one starved to death. I did not check if it was a legendary swimmer by then. Mommy, a champion with over fifty notable kills, didn't shed a tear as far as I know.

You could try altering routes by making the river bottom a restricted zone. I also add ramps to any river, and restricting the river bottom (plus maybe a few bridges) usually does the trick for me.

I regard this as good motivation to teach your soldiers how to swim. Even dwarves so modestly skilled as [no title] swimmer tend to fare, well, swimmingly against aquatic beasts.

I've noticed that dwarves caught above ground -that is, not even outside- will stop all they are doing and not move if you forbid dwarves from going outside. Those actually outside move fine; only those in an inside aboveground area seem paralysed.
