God, way to talk around the question that was asked people. I think out of all the answers here only one or two are about the question itself.
Anyways, speardwarves sparring is always dangerous. Their critical hits will always impale internal organs, so it's way, way deadlier than all other forms of sparring. You basically have two ways to circumvent this:

a) don't start training speardwarves without having at least access to plate armor and silver/any other 50% ore. make silver/other 50% ore spears ONLY and use plate/chainmal for better protection.

b) Don't train speardwarves at all. put them on duty, and if they are off duty switch to another weapon via the weapons menu (dwarves can carry two weapons after all) When you send them into combat switch to the spears, they can gain exp that way too and you won't face any sparring injuries that way, at least not from spears.

You probably set things wrong, because if you ask for the items it actually has a huge effect. I'd say they are rather important, but not really necessary.

Is the squad marked to "chase" enemies?

Because the usual behavior of a marksdwarf would be the following:

1.) Shoot bolts
2.) If Bolts run out go whack in melee with crossbow


Since there is no path to the enemies they are stuck at #2

dtutil will only work on 40d. the 40d11 exe does not have the same offsets to modify, thats why it won't work.

Don't worry about cave adaption too much as a new player. It's rather harmless save for some vomiting.

The higher the fps rate the faster the game runs. having FPS at 100 or 200 is a great way to speed up the (usually) boring first year for example.

If no goblins have shown up yet at this point there is a good chance that they never will.

you dont have a bag filled with sand. collect sand first, and make sure the bags aren't forbidden or something if you already have sand bags.
