City-states aren't limited to single cities. In fact they can have large territories, smaller settlements and farming villages and military outposts are supporting their city-state V:

The Legionaires launched an attack on Switzerland through the Alps. Treacherous, snow-covered mountains were almost a lethal obstacle for the Roman soldiers, but they've manage to gain a foothold and then obliterate the Banner's garrisons in key cities. The mountainous crossroads of Europa was now under control of Roman Republic! Unfortunately, main headquarters of the green army were destroyed during the offensive.
