I tend to do both.  If I have a biome that's easy to setup up in then I generally build close to the surface and rarely go lower.  If it is a harsher biome then I dig for the cavern and set up there.  I generally prefer the cavern forts as I hate having my dwarves live in dirt rooms.  :)

I saw this recently and it was a Troll running around in my fort tearing everything down.  My dwarves completely ignored him, so I didn't even know he was there until a cage trap caught him.  I didn't get any warnings (not sure if you're supposed too?).

I also remove aquifers, I add the ability to make a bed out of stone and cloth.  Because let's face it, if you can make a dresser out of stone you should be able to make a bed!  I also add practice workshops from the wiki, but I power them with dead vermin which if you have a cat or two adds up pretty quick.

Are these wagons?  Stairways, traps, minecart tracks, trees and boulders will block them.  If they can't leave the rest of the caravan will stay as well and eventually go insane.

well I can think of a few possible reasons.

For the bridge issue:

Check to ensure that the mechanism you want to use is available and that dwarves can path to it.  I have had the linking job cancelled because the mechanism I chose was not reachable (in my case it fell down a cistern).  The dwarf cancelled the job but I didn't notice it right away. 

Should be able to hit 'a' and see the announcment of the job cancellation. 

You can also hit try to link it to the bridge again.  If the lever is already linked to the bridge the bridge will not be an option in the list of objects you can link.

For the the trade depot:

Did you hit (q)-(r) to Request a trader at the depot?  Have you tried allowing anyone to trade and does that work?

I have the same situation going on in my current fort.  I am building a separate trade depot for them.  It's fully equipped with a marble trading depot, lots of cats, and a lava tube!

I have made it my mission to eradicate them!  Stinking goblins!

Is it frozen year round?  I had one map where it was frozen for the whole year except Late Summer.  So I mined up underneath it while it was frozen and it filled my cistern for the 5 minutes or so that it is unfrozen.

You can also run some magma underneath it to melt the ice.  That can be a huge project though depending on how available magma is on your map.

Dunno if this is a bug or just dwarven insanity. But the newest version of DF, I was embarked on a volcano for not even two minutes in real time, and my expedition leader well.She claimed herself as QUEEN! It's kind of funny.

Nah I got that last night.  I got a message when I chose my biome that my civilization was dead or dying and that my dwarves may take high positions, or something like that.  First year in my Farmer becomes Queen.  Two years in and I found out the last dwarven civ was wiped out by goblins and that my fort was the last surviving site for dwarves, at least I think it is.  Strange though as I still get migrants and dwarven traders.  Where do they haul their goods too?  lol...

Yes, I use my own unpublished mod, it has orcs as well as many other dangerous things :)
