it seems that when any noble dies it happens.
I know of no way to fix it.

anything that gets knocked out can be caught in a cage trap, demons, titans, FBs, dwarves ect.
though it's not without draw backs.  Most people cannot find the resources to sustain cage traps to their full efficiency.  Wood is a resource that needs to be used  for other projects and at 3 bars a cage, the metal smithing cost isn't worth slapping it on repeat.  then caught hostile creatures are a bitch to get rid of, so there is no viable or quick way to recycle them.  It's useful if you're in an area with a good amount of trainable wildlife, but anywhere else it's just too much of a strain on resources.

ya did all that and they still go into individual drills methink my game is bugged; got about 60 dorfs and it is still impossible for me to have an arsenal dwarf...
that may be a different bug, if one of your nobles dies, you cannot replace or create any more nobles

there are a few different things that should be done
first, you need to set their schedule to train (default) then set them to active, instead of inactive where they will do their indevidual training, but not be military dwarves.  you should also check what the number is set to which sets how many would train at the same time.  Then, at the barracks setting screen, you need to designate that barracks to [T]rain.  those are a few things that need to be in check for it to work for me.

your fortress wealth contributes to more people hearing about your fort.  all that smoothing and engraving as well as selling to merchants boosts your wealth pretty high, causing an influx of migrants.

one small caveat: if the stone to be used for the suspended wall (it still requires a stone designated, though it will never be actually used for this) happens to be on the spot that is intended for the permanent wall, it will cancel the construction of that wall, not allowing anything to happen. this is rare, but i have had it happen often enough that it confused me.
your first listed material will be the closest, so if the closest rock happens to be on the spot which you are building on, they could not move the designated rock and will cancel.  This is a huge problem when mass designating walls, one wall will be ontop of the used material and, if the material isn't used before the wall is attempted to be constructed, that segment will be suspended.

I can never get my dwarves to die fast enough
when I usually restart after the fort goes belly up from noble glitch/indestructible titan/rampaging lawgiver(though I can usually get these done easily, channeling under them and then building a floor over that tile), I just set my drawbridge to be pulled on repeat and put all the outside stock piles inside and all the inside stockpiles outside.  Unfortunately, I can't kill the last few fast enough to lose before I get migrants.
and then there's the problem when there is only one left...

you could also try leaving the last box to be walled in on the left (dwarves build from the left, you know).  This is very important to keep in mind while doing controlled cave ins/punching through aquifers
