As far as I know it's completely random.

Right now it's almost completely random (the only non-random factor is being conscious), but Toady plans to eventually implement things like reach (for instance, attacking a body part much higher than you is automatically an Impossible Strike) and response opportunities (the quintessential example is striking or jumping onto the head of a dragon as it tries to bite you).

You can kill enough that they stop spawning. It might technically count as a bug, but you can do it.

I use shields because I prefer longevity to greater power, plus this is one of the few games where shields are really work a crap.

go quest with another civ, it will improve your relations with all civs.
In 31.17 there are no other civs. No dwarves, elves, or even goblins that you can quest for. The new quest system, as well as the civ building system, hasn't been implemented, so it seems they just don't exist. Thus making all adventurers, whether elven or dwarven, the first mutation into their new species.

There are still multiple human civilizations.

I just vanquished two giants with 30+kills, with only a shattered cartilage in my left ear. Any way to fix that, or should I wear it proudly as my first battle scar?

If quick travel doesn't fix it, nothing in Adventure Mode can because you can't get doctors to help you.

Scarring actually seems to be more frequent in this version; my previous adventure got a scar from nearly every injury.

Help i don't know what to do ;( do i have to retire? My char spine is heavily bruised it's the only wound i got, i killed tons of evil night beasts and this troll i killed ruined my spine i can no longer stand up i fall, what do i do can sleeping forever heal it?

Have you tried quick travel? If that doesn't work a crutch might.

It's based on party size (easier to find to ambush), reputation (you're making more enemies), or both.

I prefer to try and disable by attacking the legs (which slows them down a lot) or arms in use, though if I'll go for the head if square and not to hard to hit.

I usually wield around 8 shields. I just come up to enemies, wrestle their weapons out of their hands and kill them with the said weapons. It's much more effective than trying to kill them when they are armed. I sometimes wrestle shields out of their hands too so I get more shields with each bandit I get.

I keep forgetting, does more shields even make blocking more likely?

Or, start adventurers that raid other towns, then you will always have a quest because each group wants an eye for that previous eye, and maybe a few teeth too.

But doesn't everyone in a given civilization automatically stop talking to you once you steal from or attack any of them?
