Smaller enemies are small enough that any attack which damages the skull probably damages the brain, but not always. It happens more often when, say, one adventurer is fighting a titan. I can't tell you how often I've had to stab, slash, or bash a titan's head hundreds of times, shattering the skull plenty, before it finally took the hint and died.

I think that nonlethal brain damage and negative effects of shattered skulls should both be implemented.

Again, you do.
The room you're in isn't yours. It's a cave that looks like it was modeled after it. Where your bed was is a pile of rags, straw, and plant bits. Where your desk was is a flattish rock, with a single piece of paper on it. Where your dresser and bookshelves were are a rock with a wooden slab on top and a few cubbies carved into the wall. Where you had a lamp on your bedside table is a big crate with an ancient lantern on it, a little bit of smouldering mossy stuff in it. Past the oddly bright light of the smouldering stuff, you see some weird shapes out a hole which replaces your door.
Glancing at the GameHand, you note that it looks different.

Oh, and it now seems to be a tattoo or something on the back of your hand.
And while we're on you, you're a big lizard thingy with some short, kinda patchy fur. Imagine four parts iguana, one part fox, two parts rat, and one part seagull. That's you, but you're on two legs.

Actually, shattering the skull does NOTHING--it's only the brain that matters.

I suppose that making skulls tougher could make sense, but you CAN get fatal brain damage from a punch.

Seems a wee bit past 1400s-era tech...
Maybe hook ballistae up to pressure plates or levers, but that's about it.

Maybe. I don't know though, it would make it easier I suppose.
And easier to expand. If you added, say, an imp, you could give it a "flame" or "dark" ability without changing every entry affecting fire or evil. Maybe add more creature tags, like "summoned." Make undead and balrogs "dark" creatures and have the paladin's holy aura affect "dark" creatures.

A standard set of status effects would possibly be better- like blinding (anti ranged), silencing (anti magic), paralysis (anti melee), fatigue (turnskip), burn,poison,bleeding (damage per turn).
That's not much like what I'm suggesting, but it makes sense.
