In my experience with eyes i would think to see.
 "Urist Mcdwarf accidental pokes self in eye"
 "Urist Mcdwarf gives into the pain"

I have no idea what sort of pain they would go through to get their eyes ripped out.

From what i have witnessed i have never seen a goblin make anything, only attack caravans.
Thus it is more likely humans (or elves) made the dwarf hair amulet. So we should hate the humans (and elves).

Then again i have never witnessed humans and elves ever making anything, therefore !DWARFS! must have made it. 
So we should hate the dwarfs. [/logic]  :P

Steel crossbow would be a waste of steel. Material doesn't affect the damage of the actual shot, only useful for clubing, like a bad war hammer, bone crossbows work just fine. Besides silver should be the best possible material.

That is still true.

And this is not a bug. Sometimes civilizations wipe each other out. That's how war works.
But what race could possibly destroy all the dwarfs?

Are you sure you put it in the right file.
Which file did you add it to?

Otherwise the it might only take effect after you start a new game.

You can stop them being careless with their prizes.
Step 1. Dig hole (20ish Z-levels deep)
Step 2. "i" for zone at the top of the hole assign a pit/pond
Step 3. Still in the "i" menu assign all your cats to the pit.
Step 4. Now you cats will kill any vermin they find and deposit the remains in the pit the pit is deep enough for the miasma not to reach the surface.

Point 1.
Your least armored (And the ones with the tiny weapons) can run the fastest. This isn't necessarily a problem as you want to be able to catch thieves but if you plan to intercept any goblin ambushers You will find you light weights brawling with the big boys long before the rest of you military can catch up.
Regroup with the move command near the point you want them to be at but out of sight of the enemy (Otherwise they will attack anyway) wait for everyone to arrive  and then attack.

Point 2.
Archers are assholes, you will be generally unable to match them with your own archer force as when goblins come, they come in numbers. bolt/arrows can do some damage perhaps not short term but generally causes problems after battle (With some of the "give water" bugs it's best to keep dwarfs out of the hospital ;) ). You are better off sending your slower more armored dwarfs to the charge although i avoid sending my legendary ++ Axe dwarfs out first at the risk of a the lucky (Or really unlucky) shots.

Point 3.
Fortifications make your archers invincible to melee attacks, they are awesome.

"d"esignate then
"j" to dig a downwards staircase. (At this point you have only dug half the staircase the half going down)
Then go down one Z level (Shift + < or > to navigate Z levels)    Then "u" for up staircase (below your down staircase)
