This sounds an awful lot like something I've noticed on my many glacier embarks: I can dig stairs down through ice just fine, but when I get to the LAST layer of ice before hitting rock (i.e. I have a rock floor and ice walls), the dwarves just leave a hole instead of digging stairs and all subsequent attempts to build stairs in that area, no matter what material I select and where it is, give me "cancels blah blah; cannot find path". I've always had to use ramps to get down through ice because of this. Once you are down there though it's no problem. Try digging a second access down using ramps, dig into your stairwell, build the stairs, then wall it up again. Did that make any sense?
This makes no sense at all but i can confirm that it is true - i noticed the same both in frozen aquifers and in glacial embarks. Use ramps and channels to get to the lowest layer of ice.
