Well, even in Mio's plot there were at least two bosses with multiple targets (Mus Vertex Horde and Taurus Vertex/Bovus Vertex Duo). So even if you prepare to nuke one target, there can and probably might be multiple targets.

Yeah, as you said, more or less. As far as I know, Mio's Fairy mechanic is the only mechanic that allows for new aspects for levelling weapons, all other levelled weapons can only increase in damage or improve a current aspect.

It depends. You may choose to level equipment with you or you may choose to leave it to be upgraded with crafting. The former option works with more unique weapons such as Mio's Hero equipment or Hunter weapons for their individual Hunters (>.>).
