It's not that he has ambiguous gender.
It's that he outright LOOKS LIKE A GIRL. And it's not that he doesn't tell people, it's that nobody ever asks because they think it's obvious he's a girl and he never has the heart to correct them until they start confessing to him.
Also, it will once you think whether or not it's worth dating him even if he is a guy simply because he's basically the perfect girl otherwise

And, of course, Ciel looks so girl-like the only possible way of telling he's a man is by actually having a look. Seriously, this is Ciel's appearance. Or, well, close to it. He obviously looks a bit more like Pyrrha than that with the whole ponytail thing, but yes, he basically resembles a cute anime girl.
Violet shall purchase Ciel's butler services. Or maid services.Unintentional, I was intending on a mixed Greek-French name to further the 'Pyrrha/Jaune child' joke.
Also, Elf, are you are aware that the protagonist of Black Butler is named Ciel and makes a very convincing girl when dressed up, or was the similarity unintentional?
