It's, firstly, an apology. An apology for everything Yukari ever did to Cado; scream, shout, treat him badly, everything.
Secondly, it's a full explanation of why she acts like that.
Essentially; she's had to deal with being bullied about her prosthetics from the age of five. Then, her father died in a car accident when she was six, earning her more bullying; not helped by Fuuka being in and out of the hospital. Her mother became distant as a result towards her and Fuuka, forcing Yukari to take the role of carer for Fuuka.
All this lead to just more bullying and accusations that she was too weak to deal with this.
Eventually, she even nearly lost Fuuka to a bout of bad illness, two years ago. Her mother barely reacted to it and Yukari couldn't even cry without being laughed at.
To compound issues, Haruka, her best and for a while only friend, was ill for a while too, and nobody was there to stick up for Yukari. She had to confide into her sister, which is where she explains that her violent reaction to Cado was because she had only opened up to her sister and Haruka.
The Cado thing hurt her even more; she was scared Cado might die.
Thirdly and finally, she apologizes again and promises to try to be nicer and less of a burden.
As Cado reads this, Yukari unconsciously grasps his arm softly, murmuring nothing coherent in her sleep.
