Thieves!?

A masterwork of <dwarf name> has been lost! What does this mean? is there a thief in my fortress? Is there a ghost roaming my fort? Or did some idiot dwarf throw it in magma?

Really simply, a hidden Goblin walked into your fort, found this item, took it, and left the map with it.

Place some chained animals by your stockpiles to defend it, or move them somewhere safe.

Was it an artifact?
The only way those can get lost are by thieving.
Only Masterwork items however, many things could have happened.
An engraving? Did you order someone to dig on an engraved tile?
Did the engraved tile become covered by magma? Overgrowth of plants?

Goblins send babysnatchers. Kobolds are back in DF2012.

It's treated as completely normal because this is Dwarf Fortress.  There's absolutely nothing wrong with surrounding yourself with a wall of flames, only to later realize that you're surrounded by a wall of flames.
There's nothing that can't be solved by hurling fifteen roc birds at it.

They're invisible unless a Dwarf or animal walks nearby them.

By the way,

Could it be that you're producing (wooden/bone/metal) bolts at high quality?

If you have a legendary woodscrafter and you'll get him to do some mass bolt production, he might make several masterpieces.
The typical thing about bolts is : you shoot them and the legendary crafters don't want their bolts to be used!
I mean.. a nice looking bolt is meant to go on display.. right? :p

A "masterwork lost" message means that the item was destroyed, not stolen. Check the profession of the dwarf that made the masterwork - if it's a Cook, then it was probably a prepared meal that rotted or got eaten by vermin.

"Masterwork lost" can also mean a marksdwarf shot a vulture, the bolt lodged firmly in the wound and then it flew off the map with the bolt still in it. This also happens if the enemy creature with a bolt in it is submerged in magma or atomsmashed. The way I run a fortress, this happens basically all the time and I deal with it by giving weaponsmiths nice housing and making sure most of the bolts are made by low-skill apprentices.

There's also a bug where food claimed by dwarves is not unclaimed when the owner dies and never put back in a stockpile. If your military has a high turnover rate then lots of meals will rot for no reason as Quietust mentioned.
