Does it also happen when they channel dirt tiles or just stone? If not it would appear they're getting whacked by the stone falling on them as they descend to the next zlevel with it.
This is going to make massive excavations a nightmare if the dorfs don't prioritize standing on an adjacent tile if available. To tell the truth I've never paid enough attention to be sure they don't already but its probably high time I do

I see that you stated this happens when designating c[h]annel. Does the same apply for designating an upward [r]amp?

Just tested, and no, because they dig upward ramps from the side.
Starver:
This might invalidate (or make actually dangerous) my well-digging method. Well, I'll work that one out, I suppose.
