I'm just going to say this, then: It'll be tough, but you can do it. Don't give up, willpower is practically the only ally you have left. So don't be disheartened, just keep trying and it will work out.
I'd suggest going to see your doctor again for recommendations about losing weight and maybe starting a diet, because frankly all the physiotherapists and excercise scientists I know wouldn't dare touch you without kid gloves, a tentpole and a writeup from your GP.
