Anyone else get killed by accidentally attacking their companions?

Never died, but I have ended up getting attacked by them.

Try to calm them down through the talk menu. If they refuse then slaughter them I guess.

Never forget, you have the capacity to yield, drop your weapons, and get on the ground and wait until they give up. So long as the fight hasn't progressed to no-quarter.

No, but I've had to kill one of my newer companions after he attacked my crossbowman. I think it had something to do with me assassinating a bandit captain, but I have no idea why. Perhaps the crossbowman accidentally shot him.

There is no friendly fire, so I doubt it was an accidental shot.
