That's not the brain though, the statement was "You don't know pain until someone twist a spear in your brain"  A spear being twisted in one's brain wouldn't cause any pain in the brain, so please refrain from from watching Citizen Kane with Lois Lane or singing in the rain in Maine, it's okay to feign watching Citizen Kane with Lois Lane, but not in Spain, and definitely not on a train.

What do you mean, that he didn't feel pain?  IIRC he was unconscious for several minutes, maybe it stopped hurting by then, something tells me even without pain receptors in the brain, that'd hurt.

Spelling it out: the vast majority of the time when you have a spear in your brain, you're dead.  Hence, having a spear twisted around in your brain usually doesn't hurt a bit.

But, Phineas Gage is an exception to the "dead" rule.
