I Think I Know Why Underworld Spire Entrances Sometimes May "Not" Spawn

Once you get into underworld via spire, going far enough to unload chunk hosting door will result in doors disappearing. What if this happens even if you are above underworld, like surface zone?

Does anybody know how to become an elephant man in adventure mode? I literally did all advanced world settings possible to increase chances.
