He got his hands on the new Dwarf Fortress release and played it so much that he forgot to eat and died.

Stone on ramps/stairs sometimes breaks pathfinding in unintuitive ways. In my last fortress I had three stairs in a row. Two of them had stone deposited on them from subsequent mining, and I started occassionally getting cancellation messages from dwarves needing to traverse the stairs to complete their job. But not always. Some dwarves could find their path with no difficulty. Others couldn't. My best guess is that it's a factor of where the dwarf if relative to the stairs relative to where he needs to go. With construction, it was easy enough to simply to wait a few moments then re-assign the job, and it would generally pathfind correctly. With eating or drinking it's not so simple.

Keep objects off of stairs and ramps.
