My king died of old age several years after coming to my fortress. No new king has ever been appointed, despite he and the consort having quite a few children, several of whom were born off-site and are (I assume) quite old.

There's not currently any code to handle succession during normal gameplay.  This is why if the liason dies he dead forever.
