Actually, yes. You can accidentally, um, do bad things during fights. The 1d1000000 is to determine how many kids the woman has--there's a 1 in 1,000,000 chance of having quintuplets.

No, you can take 200 damage as a minimum. It's 200d6, not 6d200.

Oh, whoops. That's a lot less doable. But then, with the shit that happens in D&D at epic spellcasting levels tanking 200 damage is still scarily plausible.
