No, it's not doing anything in real-time that would eat CPU cycles.  You pause Dwarf Fortress, then switch to Dwarf Therapist and click "Read" so it reads DF's memory, then make any changes you want, click "Commit" so it changes DF's memory, then switch back to DF and unpause.

You don't even need to pause DF, though you may miss something happening that you want to know about. Just throwing that in. But, I second using DT, DF is almost unplayable without it.

Yeah but a real dwarf is aware that since those dwarves are likely to in a gruesome (often magma related) way that it's a waste of time and instead uses DT.
