Constructed walls and channels

If you build walls underground and then channel through the tile above them, you'll get rocks in the same tile as the wall. This is probably a bug, because as far as I can tell your dwarfs have no way of getting to those rocks.

Possibly related:

If you construct a fortification on z-level x, then on z-level x+1 (above it) construct a floor using a rock block, and then deconstruct the floor, you end up with a block sitting in the middle of the fortification on z-level x where the dwarves are unable to reach it.

(I meant to build a (F)ortification on top of the existing one, not a (f)loor.)

Hmm, would the correct behaviour for channelling be no rock at all, or a rock sitting on top of the wall?  I'm guessing no rock.  For deconstruction it would be a rock on top of the wall?
