Blundered into some farm animals on the way back to report a named giant-lion kill; got mobbed to death.
Apparently, the next village over from mine has issues with my starting point. Assholes.

And I was just beginning to build up my first undead army, too! Mortally wounded with a quick hail of bolts before my undead dingoes slaughtered them.

I was marching along, and I found an ettin.
Basically, I fought through many kobolds humans, dwarves, elves, you name it.
I lopped off one head, then he got really mad and broke my ribs, then stomped me throughly..........
He died, as did i..........
Then my companion RANDOMLY DIED.
BULLSHAT.

Got attacked by two spore colonies, killed both but got poisoned by their toxic clouds - which made me stunned/unconscious for one step, and sober for another step, then all over again. I fell into a river, got stunned again and drowned. Shit/

Painfully. And usually involeing arrows, broken ribs, or fire/magma.

Do you mean crawling stumps from Genesis found in marshes? I am pretty sure they lash with branches and don't kick :D.

I attacked a Reindeer.  Then my companions turned on me.  I outran all but one, but managed to kill him after I tossed a gauntlet that knocked him out.  Thinking I was good, I started heading back to the edge of the site so I could fast travel.  All of a sudden, I was attacked by another Reindeer.  I took him out without to much trouble, butchered him, and headed on my way.  I thought it was weird that attacking a Reindeer would anger my companions and another Reindeer, but I didn't have much time to think about it before this happened:

I'm still not sure what caused this.  I guess I became a public enemy, or whatever, when I attacked that Reindeer.  Live and learn.
