Dwarf hurt for no reason?

i am on a freezing map that i havent seen an animal on yet. I had just started. But when i had only dug about 15 sqaures in i noticed a message saying cant give water needs bucket. Then i noticed that my carpenter suddenly had injured his head neck brain nose mouth and both eyes and has every bone from his ankles down broken. I have no idea how this happpend.

If you're using the newest version, it might be due to the temperature outside being much too cold for his equipment to protect him from frostbite.
