Ive seen this happen many times - Its just the Injury system being tricky right now. From my experimentation it has something to do with things Giving Into Pain, Having A Limb Severed, And Bleeding Out All At The Same Time.

And from a kind of mass Injury to several smaller places being misinterprited as being far more powerful than it actually is. For example:

You have discovered a river.
You slam into an obstacle!
Your lower body takes the full force of the impact, bruising the muscle and bruising the stomach!
Your right lower arm takes the full force of the impact, shattering the bone!
Your head takes the full force of the impact, bruising the muscle, jamming the skull through the brain and tearing apart the brain!
You have been knocked unconscious!
Your left lower leg takes the full force of the impact, shattering the bone through the conger eel leather chausse!
You have died after colliding with an obstacle.

Notice how my leg managed to break before i died? In the game, all it said was:

Your left lower leg takes the full force of the impact, shattering the bone through the conger eel leather chausse!
You have died after colliding with an obstacle.

But in the game log it told me the whole thing.

Its a bit weird.

I have had the message 'X has been struck down' when it's meant to say 'x has bled to death'. I wouldn't be surprised if it was instant bleed-out.
