Haven't had one do it again sadly, but yes, that one did.

I know I had some world gen parameters that were producing things like water capping a volcano on unpause and once had ice do this. Not sure what happened to make one erupt but I know part of the volcano was adjacent to a glacier biome.

Could it have been ice melting?

I'm not sure how it would have triggered the eruption, I remember before I cleared some folders out I tried it again and it erupted consistently, but I don't think I had dfhack at the time so I didn't know how to figure out what was going on as well as I do now.
