Why are these so hard.

I find getting the underlying structure a great help. Put in all the bones, figure out how the thumb joint figures in, work out where the knuckles sit in relation to one another... and that's honestly most of it. Then it's just practice.

Re: Hands

One thing to keep in mind is proportions. Unless you're going stylized, remember that a flat hand is about as long as the person's face is tall, and (aside from the middle fingers) the palm is about 60% of the length and the fingers 40%.

Also, the thumb runs roughly the length of the palm, with the base being about 1/2 the length of the palm (but not quite distinct from it) and the two segments that are distinct and stick out being about 1/4th each

As for shoulders - hard? I find them fairy easy. Just mark on the collarbone, then connect the ends to the proper places.
