If exposed to magic, one possible negative effect is the dwarves loins becoming sapient and talking to them. They get their own separate soul and decide if the dwarf can get pregnant/impregnate their spouse and dwarves can chat with them, but otherwise get no control over the dwarf.

Most dwarves would dislike this and do almost anything to silence their loins, but then there will always be "that dwarf" who takes immense pride in their new talkative friend and eagerly introduce it to others.

The nut spirits will get upset if a dwarf wears pants or underwear. This will not change the preference of the dwarf for wearing pants. The only solution is for dwarves with sentient junk to compromise by wearing skirts, which will be defined with an [OPEN_ENDED:BOTTOM] tag so the game recognizes they fit this criteria.

Sentient crotch parts can also commit crimes. This can result in some unpleasant times for a dwarf if their loins commit treason or murder and are sentenced to a hammering.
